To meet eligibility criteria, you must have a prognosis of one year or less and reside within a 75 km driving distance of 175 First Street Collingwood, Ontario, Canada, including the following areas: Alliston, Angus, Barrie, Collingwood, Creemore, Durham, Elmvale, Flesherton, Innisfil, Markdale, Meaford, Midland, Mono, Orillia, Oro Medonte, Owen Sound, Penetanguishene, Shelburne, Singhampton, Stayner, Thornbury, Tottenham and Wasaga Beach​
